Insurance and Fees for Services

The Center is home to many individual private practices. Please refer to the list below for in-network insurance plans for each practice.  Out-of-network patients are welcome but you will be expected to pay in full at the time of the visit. The Center will then file your out-of-network reimbursement claim on your behalf and your insurance company will send any out-of-network reimbursements directly to you. If your doctor cannot file your claim for you, we will give you the necessary paperwork to submit to your insurance plan for reimbursement.

All routine laboratory tests performed at the Center are processed by Quest Diagnostics. Quest accepts all major insurance plans and will process your insurance claims. Quest offers discounted laboratory test fees to patients without health insurance. If you and your provider decide that you need specialty labs that are not covered by insurance plans, fees for these specialty labs will be discussed at your appointment.

If you have health insurance, please provide your current information when you schedule your first appointment. There are far too many insurance plans for us to know the details of any one plan. We ask you to assume responsibility for understanding your insurance coverage by calling your insurance company directly with any questions you may have about your particular coverage. Please bring proof of insurance and a photo I.D. to each visit. You will be expected to pay your co-pay and/or deductible, as well as any outstanding balance at time of service.

Patients without insurance will need to pay in full at the time of service. Checks, cash, Visa, Master and Discover cards are accepted.

If your provider is not listed below, he/she is not in network with any insurance plans. These providers will provide you with a receipt for service which can be filed for reimbursement if your particular plan permits.

Note: Many insurance plans noted above provide routine eye coverage through vision service plans. These are separate plans that Dr. Tesser is not in network with. For example, Dr. Tesser is not in network with VSP, Davis Vision, or Spectera. Check and make sure your insurance plan covers routine eye examinations before assuming your visit will be covered under your insurance plan. For patients who are out-of-network, Dr. Tesser offers a reduced eye exam rate of $125 which includes the refraction.
